Enzyme inhibitors and monoclonal antibodies can be used in the treatment of disease.

  1. (a)Mevinolin is an enzyme inhibitor that can be prescribed as a drug to reduce the concentration of cholesterol in blood plasma. High concentrations of cholesterol in the blood have been linked to an increased risk of cardiovascular disease. Mevinolin acts as a competitive inhibitor of the enzyme HMG CoA reductase. This enzyme catalyses one of the first steps in the synthesis of cholesterol, as shown in Fig. 6.1. [Figure 6.1] Explain how mevinolin inhibits the enzyme HMG CoA reductase.[3]
  2. (b)Outline the use of monoclonal antibodies in the treatment of disease.[2]

Mark scheme

Marking points from the official mark scheme (May/June 2018). The mark or mark type is shown in brackets.

  1. [3] three from:
  2. same / similar, shape as, HMG CoA / substrate ;
  3. complementary (shape) to active site (shape);
  4. binds / attaches / fits into / active site (of, enzyme / HMG CoA reductase) ;
  5. HMG CoA / substrate, cannot bind (to active site) ; A no / few, enzyme/substrate complexes form I product not made
  6. mevalonic acid production, decreases / stops ;
  7. AVP; e.g. increase in mevinolin concentration increases inhibition / ora
  8. [2] two from:
  9. treatment of disease
  10. I statements about locating or diagnosis of disease
  11. specific / targeted, therapy / treatment ; A specific to diseased, cells / tissue A examples
  12. bind to / recognition of, receptors / antigens, on cell surface (of diseased cells) ;
  13. kill the cell by stimulating the immune system / AW ; A correct immune responses e.g. stimulates phagocytosis / activates B-cells
  14. attach, radioactive substance / drug (to treat / kill diseased cells) / AW ;
